The Youth Support Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll partner with care teams, families to keep work moving and ensure key care plans are accurate and up to date. You'll balance quality and pace, using judgement to triage requests and escalate risks at the right time. Strong performance is shown through consistent delivery, accurate records and effective use of clinical systems (varies by service) to track and communicate progress.

What you’ll do day to day

  • Monitor wellbeing and escalate clinical concerns promptly and appropriately
  • Support medication safety and infection prevention practices where relevant
  • Contribute to quality improvement and risk management activities
  • Communicate clearly with patients, families and the multidisciplinary team
  • Promote dignity, privacy and culturally safe practice in all interactions
  • Conduct assessments and contribute to care plans, updating notes accurately

Skills and tools to learn

Core skills: risk assessment, clinical communication, empathy, documentation, teamwork

Software/Platforms: clinical systems (varies by service), telehealth tools, Microsoft 365

Methods/Knowledge: infection prevention, care planning, medication safety (as applicable)
