The Support Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll coordinate with clinicians, care teams, keeping care plans well managed and easy to access when needed. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. You'll be successful when stakeholders feel supported, work is well documented, and systems like Microsoft 365 are kept reliable and current.

What you’ll do day to day

  • Monitor wellbeing and escalate clinical concerns promptly and appropriately
  • Communicate clearly with patients, families and the multidisciplinary team
  • Conduct assessments and contribute to care plans, updating notes accurately
  • Contribute to quality improvement and risk management activities
  • Support medication safety and infection prevention practices where relevant

Skills and tools to learn

Core skills: clinical communication, risk assessment, empathy, documentation, teamwork

Software/Platforms: telehealth tools, clinical systems (varies by service), Microsoft 365

Methods/Knowledge: infection prevention, medication safety (as applicable), care planning
