Rehabilitation Support Worker
A Rehabilitation Support Worker is a detail-oriented professional who helps deliver reliable outcomes in day-to-day operations. You'll work with patients, families and maintain clear handover notes so decisions and actions stay on track. You'll balance quality and pace, using judgement to triage requests and escalate risks at the right time. You'll be successful when stakeholders feel supported, work is well documented, and systems like Microsoft 365 are kept reliable and current.
What you’ll do day to day
- Monitor wellbeing and escalate clinical concerns promptly and appropriately
- Promote dignity, privacy and culturally safe practice in all interactions
- Complete documentation and handovers to ensure continuity of care
- Conduct assessments and contribute to care plans, updating notes accurately
- Deliver safe, person-centred care in line with clinical standards and scope of practice
- Contribute to quality improvement and risk management activities
Skills and tools to learn
Core skills: teamwork, risk assessment, empathy, clinical communication, documentation
Software/Platforms: Microsoft 365, clinical systems (varies by service), telehealth tools
Methods/Knowledge: infection prevention, medication safety (as applicable), care planning
