Family Support Worker - Senior
A Family Support Worker - Senior is a outcomes-focused professional who helps deliver reliable outcomes in day-to-day operations. You'll coordinate with clinicians, patients, keeping clinical notes well managed and easy to access when needed. You'll take ownership of complex work, coach peers and drive continuous improvement across workflows. You'll be successful when stakeholders feel supported, work is well documented, and systems like Microsoft 365 are kept reliable and current.
What you’ll do day to day
- Deliver safe, person-centred care in line with clinical standards and scope of practice
- Communicate clearly with patients, families and the multidisciplinary team
- Promote dignity, privacy and culturally safe practice in all interactions
- Conduct assessments and contribute to care plans, updating notes accurately
- Complete documentation and handovers to ensure continuity of care
- Support medication safety and infection prevention practices where relevant
- Coach others, share knowledge and improve processes to lift team capability and quality
Skills and tools to learn
Core skills: risk assessment, clinical communication, empathy, documentation, stakeholder management
Software/Platforms: telehealth tools, Microsoft 365, clinical systems (varies by service)
