The Family Support Worker - Junior supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll work with families, care teams and maintain clear clinical notes so decisions and actions stay on track. You'll focus on getting the fundamentals right, with guidance from experienced colleagues and clear checklists or templates. You'll be successful when stakeholders feel supported, work is well documented, and systems like telehealth tools are kept reliable and current.

What you’ll do day to day

  • Monitor wellbeing and escalate clinical concerns promptly and appropriately
  • Complete documentation and handovers to ensure continuity of care
  • Support medication safety and infection prevention practices where relevant
  • Conduct assessments and contribute to care plans, updating notes accurately
  • Communicate clearly with patients, families and the multidisciplinary team
  • Promote dignity, privacy and culturally safe practice in all interactions
  • Follow established procedures and checklists, escalating issues early and learning from feedback

Skills and tools to learn

Core skills: risk assessment, empathy, teamwork, clinical communication, learning agility
