As a Disability Support Worker, you bring a service-led approach to delivering services and supporting teams to meet their goals. You'll coordinate with clinicians, care teams, keeping handover notes well managed and easy to access when needed. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. Strong performance is shown through consistent delivery, accurate records and effective use of clinical systems (varies by service) to track and communicate progress.

What you’ll do day to day

  • Contribute to quality improvement and risk management activities
  • Deliver safe, person-centred care in line with clinical standards and scope of practice
  • Conduct assessments and contribute to care plans, updating notes accurately
  • Communicate clearly with patients, families and the multidisciplinary team
  • Support medication safety and infection prevention practices where relevant
  • Promote dignity, privacy and culturally safe practice in all interactions
  • Complete documentation and handovers to ensure continuity of care

Skills and tools to learn

Core skills: clinical communication, risk assessment, teamwork, documentation, empathy

Software/Platforms: Microsoft 365, clinical systems (varies by service), telehealth tools

Methods/Knowledge: care planning, infection prevention, medication safety (as applicable)
