Disability Service Officer
As a Disability Service Officer, you bring a process-savvy approach to delivering services and supporting teams to meet their goals. You'll coordinate with clinicians, patients, keeping risk assessments well managed and easy to access when needed. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. Success looks like timely follow-through, strong communication and confident use of tools such as telehealth tools.
What you’ll do day to day
- Promote dignity, privacy and culturally safe practice in all interactions
- Complete documentation and handovers to ensure continuity of care
- Conduct assessments and contribute to care plans, updating notes accurately
- Monitor wellbeing and escalate clinical concerns promptly and appropriately
- Support medication safety and infection prevention practices where relevant
- Contribute to quality improvement and risk management activities
- Communicate clearly with patients, families and the multidisciplinary team
Skills and tools to learn
Core skills: documentation, teamwork, empathy, risk assessment, clinical communication
Software/Platforms: telehealth tools, clinical systems (varies by service), Microsoft 365
Methods/Knowledge: care planning, infection prevention, medication safety (as applicable)
