The Community Rehabilitation Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll work with families, government agencies and maintain clear progress reports so decisions and actions stay on track. You'll balance quality and pace, using judgement to triage requests and escalate risks at the right time. Success looks like timely follow-through, strong communication and confident use of tools such as secure record-keeping tools.

What you’ll do day to day

  • Develop and review plans with clients, focusing on practical, achievable outcomes
  • Support community engagement activities, programs or events where required
  • Contribute to compliance, quality standards and continuous improvement activities
  • Maintain accurate and confidential case notes and reporting documentation
  • Coordinate with service providers and agencies to support wraparound care
  • Manage risk, safety and duty-of-care responsibilities in complex situations
  • Provide client support through intake, assessment and referral pathways
