Community Rehabilitation Support Worker
The Community Rehabilitation Support Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll work with patients, families and maintain clear handover notes so decisions and actions stay on track. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. Success looks like timely follow-through, strong communication and confident use of tools such as clinical systems (varies by service).
What you’ll do day to day
- Conduct assessments and contribute to care plans, updating notes accurately
- Communicate clearly with patients, families and the multidisciplinary team
- Monitor wellbeing and escalate clinical concerns promptly and appropriately
- Promote dignity, privacy and culturally safe practice in all interactions
- Support medication safety and infection prevention practices where relevant
- Deliver safe, person-centred care in line with clinical standards and scope of practice
- Complete documentation and handovers to ensure continuity of care
Skills and tools to learn
Core skills: risk assessment, clinical communication, empathy, documentation, teamwork
Software/Platforms: telehealth tools, clinical systems (varies by service), Microsoft 365
Methods/Knowledge: medication safety (as applicable), care planning, infection prevention
