Community Personal Care Worker
The Community Personal Care Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll coordinate with government agencies, families, keeping case plans well managed and easy to access when needed. You'll balance quality and pace, using judgement to triage requests and escalate risks at the right time. You'll be successful when stakeholders feel supported, work is well documented, and systems like secure record-keeping tools are kept reliable and current.
What you’ll do day to day
- Provide client support through intake, assessment and referral pathways
- Manage risk, safety and duty-of-care responsibilities in complex situations
- Develop and review plans with clients, focusing on practical, achievable outcomes
- Advocate for clients and assist with navigating systems and entitlements
- Maintain accurate and confidential case notes and reporting documentation
Skills and tools to learn
Core skills: documentation, active listening, culturally safe practice, boundaries, case coordination
Software/Platforms: Microsoft 365, secure record keeping, case management systems
Methods/Knowledge: strengths-based practice, referral pathways, risk and safety planning
