As a Community Care Worker, you bring a service-led approach to delivering services and supporting teams to meet their goals. You'll coordinate with clients, service providers, keeping intake notes well managed and easy to access when needed. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. Strong performance is shown through consistent delivery, accurate records and effective use of case management systems to track and communicate progress.

What you’ll do day to day

  • Maintain accurate and confidential case notes and reporting documentation
  • Coordinate with service providers and agencies to support wraparound care
  • Contribute to compliance, quality standards and continuous improvement activities
  • Support community engagement activities, programs or events where required
  • Advocate for clients and assist with navigating systems and entitlements

Skills and tools to learn

Core skills: boundaries, documentation, active listening, culturally safe practice, case coordination

Software/Platforms: case management systems, secure record keeping, Microsoft 365

Methods/Knowledge: risk and safety planning, strengths-based practice, referral pathways
