The Case Worker supports consistent delivery by coordinating tasks, information and stakeholders across the workday. You'll coordinate with government agencies, families, keeping progress reports well managed and easy to access when needed. You'll own recurring tasks end-to-end, manage competing priorities and recommend small improvements that reduce friction. Success looks like timely follow-through, strong communication and confident use of tools such as case management systems.

What you’ll do day to day

  • Advocate for clients and assist with navigating systems and entitlements
  • Develop and review plans with clients, focusing on practical, achievable outcomes
  • Contribute to compliance, quality standards and continuous improvement activities
  • Maintain accurate and confidential case notes and reporting documentation
  • Provide client support through intake, assessment and referral pathways
  • Manage risk, safety and duty-of-care responsibilities in complex situations
  • Coordinate with service providers and agencies to support wraparound care

Skills and tools to learn

Core skills: boundaries, documentation, culturally safe practice, active listening, case coordination

Software/Platforms: secure record keeping, case management systems, Microsoft 365
