An Aged Care Support Worker is a detail-oriented professional who helps deliver reliable outcomes in day-to-day operations. You'll partner with clinicians, care teams to keep work moving and ensure key risk assessments are accurate and up to date. You'll balance quality and pace, using judgement to triage requests and escalate risks at the right time. You'll be successful when stakeholders feel supported, work is well documented, and systems like Microsoft 365 are kept reliable and current.

What you’ll do day to day

  • Conduct assessments and contribute to care plans, updating notes accurately
  • Complete documentation and handovers to ensure continuity of care
  • Deliver safe, person-centred care in line with clinical standards and scope of practice
  • Monitor wellbeing and escalate clinical concerns promptly and appropriately
  • Contribute to quality improvement and risk management activities

Skills and tools to learn

Core skills: clinical communication, empathy, documentation, risk assessment, teamwork

Software/Platforms: Microsoft 365, telehealth tools, clinical systems (varies by service)

Methods/Knowledge: medication safety (as applicable), infection prevention, care planning
